The residency program at Valley Pharmacy focuses on community pharmacy practice providing comprehensive patient care in a hospital based, outpatient setting. This innovative residency offers the opportunity to provide medication counseling and management to ambulatory patients discharged from Winchester Medical Center and Warren Memorial Hospital. The resident will be skilled in working collaboratively with case management and other healthcare providers by identifying barriers and ensuring compliance, thus improving patient outcomes and reducing hospital readmissions.
Upon successful completion of this program, the resident will be prepared to be an advanced practice community pharmacist embracing the opportunities for transition from the inpatient to outpatient setting. The resident will develop programs to expand clinical services into the community with an emphasis on the development, marketing, and implementation of new patient care and disease state management programs. The resident will also have the opportunity to work at the Shenandoah Valley Compassionate Pharmacy, a nonprofit facility serving low-income patients, enrolling patients in PMAP’s, dispensing medications, and providing patient counseling. Other areas of experience include leadership and management training with the pharmacy director, transitions of care with inpatient pharmacists at Warren Memorial Hospital and medication safety at Winchester Medical Center. Elective opportunities are available in different practice areas such as ambulatory care, academia, and pharmacy management.
As a Valley Health System entity there is a vast opportunity to expand on and develop selected clinical services critical to patient outcomes and the continuum of care. Staffing requirements will provide experiences such as pediatric dosing, compounding, immunization administration and drug information. The resident will be given the opportunity to complete the Shenandoah University School of Pharmacy Residency Teaching and Learning Curriculum Program. Additionally, mentoring APPE and IPPE student pharmacists will give the resident the ability to define their own teaching methods.
