To request an estimate of your out-of-pocket expenses for future services, click here. Please keep in mind that this is an estimate and actual out of pocket expenses may vary as a result of the quality and accuracy of the information provided when making the request, the actual care provided at the time of service, and the level of coverage applied by your insurance (if any) when they process your claim. Pre-payment discounts are available for services that can be scheduled in advance.

In many cases, you will be asked to fill out a Financial Information Sheet (FIS). We can provide one, or you can print one out on this page. Please fill out the form and return it in a timely manner so we can review your circumstances. This will help us make a more informed determination/suggestion of your options for resolving your bill.

Valley Health encourages patients to apply for Financial Assistance if they believe they are unable to pay all or part of their healthcare bill for medically necessary services. Keep in mind that not all Valley Health services offer the same assistance programs and not all patients are eligible for those programs.
Financial Assistance is available for those patients who meet predefined criteria. You can read more about our Financial Assistance Program on this page. The first step is to contact us to request a Financial Information Sheet (FIS). You must return your FIS timely so that we can review the individual or families’ information against the criteria. In most cases, additional documentation is required such as proof of income or a notarized letter of support. You must request this prior to your account being referred for collections.
Valley Health facilities participate in several financial assistance programs through local, state and federal government programs, including Virginia Medicaid and West Virginia Medicaid programs. To help patients and families work through the eligibility requirements for these programs we have staff to assist.
We recognize that not all uninsured or underinsured patients meet the eligibility requirements of the available state or federal programs. All of your information will be kept confidential and only used to determine possible assistance programs to resolve your healthcare bills.
